Who We Are
The Jemez Health Education and Health Promotion Program serves as a community resource to promote, educate, and develop interventions that address health issues within the Pueblo. The team is multi-disciplinary and includes a Public Health Practitioner, a Certified Personal Trainer, Health Advocates, and a Media Specialist. Programming combines evidence-based resources with Hemish knowledge to influence attitudes and behaviors that build skills to support a healthy lifestyle. In addition, this program collaborates with other POJ departments and JHHS programs to improve health services, execute health promotion strategies, and systematically deliver health interventions at an individual, group, community, and institutional level.

Embark on a healthier lifestyle with the FiTT Driven Incentive Program!
This program is dedicated to promoting a healthy lifestyle through positive lifestyle changes. Participants are encouraged to stay active and hydrated, participate in FiTT challenges, and attend provider visits to earn FiTT points. These points can be redeemed for rewards, motivating individuals to prioritize physical activity and hydration, which are both crucial for managing blood sugar levels and maintaining a healthy lifestyle. Join us on this journey toward better health and take control of your wellness and/or diabetes management today!

FiTT Driven
Incentive Activities

Water Buffalo
​Water Buffalo encourages participants to increase their water intake by drinking 80 ounces of water every day. For every 10 ounces of water you drink, you earn up to 1 FiTT point. You may earn up to 8 FiTT points per day. Log your points by clicking the icon below or you may request a water intake card from a Community Health Advocate at Public Health.

FiTT Driven Workouts
A personal workout includes any physical activity that gets your heartbeat elevated such as gym workouts, group workout classes, chopping wood, cleaning the house, or walks/runs. A personal workout is worth 15 FiTT Points. You are allowed only one entry per day. You can log your points by clicking the icon below.

FiTT Points
FiTT points will be erased annually on December 31st of every year starting with a new slate of points January 1st. This ensures that FiTT members stay motivated in taking ownership of their steps toward a healthier lifestyle.
